Sounds like failure to immediately correct for loss of one engine.

I never got to take off in a twin but did get to the point of being ready to solo in a single engine. The torque of the engine caused the plane to try to rotate and you had to step on the rudder pedal to counter it on takeoff. I can see where an engine failure on takeoff could get hairy quick on a twin.
