Something similar happened on a US Navy ship incident recently. I remember reading something about how some of the female officers on duty were not talking to each other and it contributed to the collision.
Isnt this a similar situation to the USS Fitzgerald where the female officer of the deck and the navigator werent speaking to each other?
Were they trying to parallel park the thing?
