Have you considered asking your doctor whether he's thought of setting up a "concierge practice"? It can exist alongside his current insurance-centric practice as I understand it.
For all this, my doctor of the past 35 years won't see me any more. He gets paid $85 for a visit that previously paid thousands. I don't blame him.

If you practice in an upscale area in South Florida or around the beltway, this is perhaps viable.

Most fail, and most practicing physicians do not have the capital necessary to initialize with the enormous risk of failure.
