I am pretty sure they say that in every hierarchical bureaucracy. I have never been in the military but we have used that in private industry for at least 40 years. It probably goes back to the days of the Roman Empire. Or cavemen.

The Navy and Air Force have been strong proponents of political correctness and now pay the price. No sympathy from this retired Army Officer.
I was in nearly 30 years ago...today's Navy wasn't my Navy, either. I'm not saying that officers and enlisted didn't have issues with one another, but we were always professional enough not to let them get in the way of our mission.

There were radar repeaters located at the lookout stations on the bridge wings of my ship. I don't see it as possible to miss something as big as a merchant ship!
