The following table was taken from the Wikipedia article on the FISA Court (Internal citations omitted). It is actually rather illuminating. It would appear that prior to 2016, the FISA court denied a grand total of 17 out of 38,365 requests. In 2016 and 2017, 68 were denied.

I would be really interested in knowing what the nature of those 68 denied requests were, compared to those before those of 2016. Of course, since it is a secret court, being given secret 'evidence' regarding secret requests, those of us who pay the bills for all of this will most likely never know anything about any of that. Star Chambers like this only really operate well in darkness. There really needs to be some way for having someone who actually gives a damn about the Constitution and our rights under it to be involved in this 'process'. It is quite apparent that you won't find such people within the law enforcement or 'national security' apparatus that would be qualified to do so.

General warrants from secret courts indeed have no place in a free country.

Real, non-swamp judges would have held the Carter Page warrant signatories in contempt and tossed them in jail. It is what would happen to you or me if we lied to a judge.
