Hmmm. I wonder if either of them even considered the possibility of walking up the steps of the elevator? Neither appears to be disabled differently enabled. Nah, that's too much trouble and besides they do need help; just not the kind of help they seem to think they need.

Unfortunately for Bruce Willis and other Hollywood ilk, it is a code requirement that a top exit hatch be accessible only from the top of the elevator and is secured by a latch mechanism of some sort (like a dead bolt) and also in most instances a safety switch that shuts the elevator off if the hatch is opened. This is required so that only those specifically trained to perform an elevator rescue can access the hatch and open it.
Many years ago you could find elevators with interior accessible hatches, often with thumb screws that allowed them to be opened. To the extent these still exist, they are grandfathered as far as the code is concerned, but many building managers worried about liability claims have had their older units modified with a top-only access device.
If you're curious as to why this is now in the code, think cities, projects, and kids doing dumb things like opening the hatch, crawling up to the top of the elevator, and then oops, falling splat into the pit.
