This is being driven by the insane tap-and-shuffle that doctors have to go through to actually collect their payment from third-party insurers. It is really bad with private insurance and a thousand times worse with Medicare. If we eliminated all insurance and went to fee-for-service they could probably cut staff, fire all of their collections agencies and reduce their fees by 70, 80%.
