Here is a list of government and civilian websites that require a long form birth certificate.
1.) Snyder County, PA requires a long form birth certificate to obtain a US passport for anyone 15 years of age or younger:

So, if a short form birth certificate is not sufficient evidence of citizenship in any of these cases, then why would it be sufficient to be President of the United States?

If you don’t like her or her statement, that’s fine with me, I could care less. Dr. Fukino serves at the pleasure of the Republican Governor of Hawaii and a McCain supporter, Linda Lingle.
There is zero relevance to the morally compromised republican governor of Hawaii or the stealthy liberal McCain in this issue, other than Lingle has a vested interest int he past of Hawaiian demographic fraud which aided the leftist state to swell their population counts for federal benefits. To be frank, I am not prepared to say that "we know" Obama was not born in Hawaii.
The problem is -- ironically -- that we know so little about him.
What in Gods Name is he hiding?
Why is he spending large amounts of money in order to prevent discovery of his vital records?
To a reasonable man -- with a modicum of common sense -- his behaver is mighty suspicious, to say the least.
In order to get a Real Estate license, in California, I had to not only send a copy of my birth certificate, I was required to have my finger prints scanned and sent to the FBI for investigation, along with sworn discloser's of ANY misdemeanors in my past -- including my Juvenal records.
All of the above, of course, was to make REASONABLY sure that I was honest and upright and not a con man or a cheat.
If I would have refused to disclose my past because there was something "embarrassing" -- even IF I, in fact, were an honest man -- the authorities would have refused to issue me a California Real Estate licence.
Now, if I WERE a fraud and cheat, I could hurt a few people, but if the President is a fraud and cheat: he can damage millions of people.
Why should he be held to a lower slandered than you or I?
This is the executive office of the President of the United Stes we are talking about!
