Witnesses to a fatal train crash involving an Ottawa city bus describe a scene of chaos and confusion as it appears the bus failed to stop at the rail crossing and crashed into the side of the train, killing six people and injuring as many as 30 others. Officials are still trying to piece together what happened prior to the devastating crash, which occurred shortly before 9 a.m. near the intersection of Woodroffe Avenue and Fallowfield Road.

AURORA, Colo. - Campus police are investigating an accidental shooting in which two people suffered minor injuries at the University of Colorado School of Dental Medicine in Aurora. The shooting happened Friday in a staff member's office at the dental school on the Anschutz Medical Campus, Campus Police Chief Doug Abraham said in a Monday statement. Two people in the office received minor injuries and were not hospitalized, the chief said. The staff member who possessed the gun had a concealed carry permit for it. Moments after his motorcycle struck a 4-year-old girl, an off-duty Chicago Police officer shot and killed the girl’s irate father after he allegedly attacked the officer, the Chicago Sun Times reports.
