Asymptomatic Coronavirus Patients Can Still Develop Lung Damage

I quickly learned that many patients with advanced COVID-19 disease bore none of the hallmarks of severe respiratory illness until they suddenly collapsed and died. ... [A] study from Wuhan, China, describes pathological lung changes on CT scans of completely asymptomatic patients. Asymptomatic carriage is not uncommon in other virulent infections, such as MRSA and C diff, but what is striking with SARS-CoV-2 (the virus that causes COVID-19) is that it may be accompanied by underlying organ damage.

The researchers found lesions consistent with inflammation of the underlying lung tissue (ground-glass opacities and consolidation, to use the medical jargon), which are not specific to SARS-CoV-2 infection and may be seen in many other forms of lung disease. What remains a mystery is why, despite these changes, patients do not display typical symptoms of pneumonia, such as severe shortness of breath.

About a quarter of the patients in the study developed a fever, cough and shortness of breath – but many did not. The idiosyncratic response to infection is one of several conundrums of COVID-19, such as why it targets certain groups and not others – two people with exactly the same demography and health may express the disease at opposite ends of the spectrum. The study reinforces that absence of symptoms does not imply the absence of harm.

Lack of symptoms in the face of active pathology carries a risk both to infected individuals and to the public. Current advice encourages patients to stay at home if they are asymptomatic, making late presentation to hospital and sudden death a distinct risk.

Fragments of evidence are slowly being pieced together, mainly from many small, disparate studies. The full picture will coalesce as the quality and quantity of evidence expand and refines our understanding of SARS-CoV-2.

Q Spectacular has a light (lower corn syrup) quinine water that is quite good. It is a bit pricey. I don’t really like quinine water, but this one is more than tolerable.

Yes. I think this is rampant speculation as it assumes causation that I do not see is there. How do we know the people didn’t have radiographic evidence of scar tissue before, and if it is only a 3 month old virus, how do we know that it wont resolve.

I think this is more fear mongering. In my practice I have seen a lot of lousy x rays before CoVID that was unclear as to the etiology and significance.

I am also seeing a decrease in acuity as the numbers seem to go up. People with tons of comorbidities are not dying but recovering. Bottom line? I am not sure what to think. I leave for the possibility of damage, but if we are going to freak out over the viral load being as high in asymptomatic and long term lung damage — are we going to install CT scanners in parking lots too so we can get more tests without a reason? This makes not a whole lot of sense.

Pivoting to the presser today with the CoVID task force — what I did hear is that the line is flattening in Florida and AZ. This is consistent with what my colleagues are telling me that the evaluations are starting to tail off. As for the deaths yesterday, Birx said probably back log and if you average the holiday weekend we are still right about at are all time lows.

So, are we any worse off now than before? More testing -> more hospitalization -> higher DRG for hospitals? MOre people in hospitals from elective surgeries and people coming back to hospitals? Probably.

Lets continue to watch vigilantly, and carry on. I sense we are close to the peak and should be on the downside in the next 10 days. We will let those who profit from dear continue their march toward fear.

Insanity 3.0. Based on a new study out of Wuhan, China, and this Drs anecdotal account, we should be skeerd that people asymptompatic with Covid-19 will suddenly collapse and die! I expect the special effects department is working on a video show us. *smh*

It’s not a flu, it’s a coronavirus and not the same category of disease. So treating it as such is not wise.

When HCQ came out, everyone was flipping about how it didn’t work, etc. Now it looks like it does.

There’s a lot we don’t know about the virus because it’s new but to dismiss it off hand because of that can be foolhardy.

How about filing the information under the category of, *Hmmm, that’s interesting. Let’s watch and see what comes of this.* instead of blowing off everyone’s concerns just because you don’t like them?

There’s the off chance they may be legitimate.

And just because of that, doesn’t mean that people are justifying the lockdown, mask wearing, etc. It’s something to watch and see if a pattern develops and to follow those with apparent lung damage to see if it’s long term or not.

There may be something to it and there may not, but nobody is going to learn anything by blowing it off.
