I don’t know Hawaii practices, but I can tell you in 1980 in California as an intern, the nurses would fill out most of the death certificates and then give them to me to sign. I have no idea what happened to them afterward. For the limited number of deliveries I did I have no memory of filling out anything. (I may have signed them, but I was chronically sleep deprived at the time.)
In the early 80’s it was a big deal to have a lab computer, and that’s probably the easiest medical thing to computerize.
I thought it strange that they'd send a lawyer to Hawaii to pick up a couple of copies. Seems to me the President of the United States should be able to get an attachment on an email and run that sucker through something at Langley that'd be sharp, clear, rehabilitated in every respect, etc.
That suggests there's no one in the chain of command between Obama and anybody who gets contact with him who knows anything about modern communications and data management ~ or, worse, printing.

Once again.. if it was a scanned image of a page in a book, it would be flat- no layers.
Show me the software they used to scan it and try to repeat that. Repeatability is the the most fundamental test of software. If ANYONE HERE can take this image, print it, and then scan it (with OCR or anything) and get ANYTHING CLOSE TO the 'layers' in this document I will publicly apologize online and kiss your ass on webcam. If any hawaiin freepers send away for their BC today and get anything remotely like this too.

I was referring to the alignment of characters from line to line. Yes, you are correct, characters can be misaligned on a line of text. But I was talking about the above and below alignment. As you know, each character lines up with characters (if any) above and below it, like in a column. And, of course, that is because the roller rides on a straight gear with fixed spacing.

Above, you can see that the typing in blocks 1, 2, 6a, 6c, 7a, 7d, 8, 10, 13, & 15 all align at the left margin. Blocks 6b, 9, 12b & 14 are aligned at a tab.
Strangely, 11 & 16 and 12a& 17a are not aligned.
