If you've ever had your ***** cut off and/or been executed while on holiday, you'll probably know that it's easy to offend people from other cultures. Unless you learn the ways of the place you're visiting, even the most well-meaning tourist can regularly find his oesophagus stuffed with burning goat. But surely just plain common sense and good manners will save you, right?

Nah, this is just in Iran & Turkey. Some parts of Iraq because of Persian influence. Even in those places, people are familiar with what it means in the rest of the world. Although I do know an Arab who once startled an Iranian cab driver with an innocent thumbs up. As for the thumbs up being considered bad in South America, never seen or heard of such a thing.
And don’t point with that index finger in east Asia—especially southeast Asia.
Never give a clock or a watch to an older person or someone as a retirement present - same as telling them you only have a little time left so you better watch it.

This is true. Hilariously, to prevent pointing with the index finger, I’ve seen these people point with their middle finger.
The mods can get a mite cranky when you don't warn about such things.
I've discussed this with the Iraqis. They do the thumbs up sign themselves and everyone I've talked to about this says it's not offensive there. They have said that the Iranians don't like it.
What they do recommend that we don't do is the "OK" sign. Apparently that one is bad there.
They're also not particularly hung up about the left-hand thing except for when eating. I make sure I never give or receive things with my left hand (and sometimes, that is a hassle), only to see them do it regularly.
