My husband and I just signed those forms 2 weeks ago.One for consent and one for end of life.We had to assign two people for each document and the attorney witnessed our signatures.

When Jim is better and able to handle his affairs, the first priority should be for him to designate Chris as his representative for medical purposes (medical power of attorney) and, if he has not done so, prepare other end-of-life forms that represent Jim’s wishes.
