Diabetes may begin more than 20 years before diagnosis

Early signs of type 2 diabetes can be identified more than 20 years before diagnosis, according to new research presented at this year's European Association for the Study of Diabetes (EASD) Annual Meeting in Berlin, Germany (1-5 October).

The Japanese study tracked over 27,000 non-diabetic adults (average age 49 years) between 2005 and 2016 and found that increased fasting glucose, higher body mass index (BMI) and impaired insulin sensitivity were detectable up to 10 years before the diagnosis of diabetes as well as prediabetes.

"As the vast majority of people with type 2 diabetes go through the stage of prediabetes, our findings suggest that elevated metabolic markers for diabetes are detectable more than 20 years before its diagnosis," says Dr Hiroyuki Sagesaka from Aizawa Hospital in Matsumoto, Japan.

In particular, BMI, fasting glucose, and insulin resistance were increased up to 10 years before diagnosis, and these differences widened over time.

Please look at a lower carb diet. You really don’t feel hungry between meals and skipping or delaying seems perfectly reasonable because you have no appetite.

Low carb means to my wife and I means you only eat what you burn and wind up eating a lot less in total calories.

Try it.

You are correct unfortunately. Moderation DOES NOT WORK. You have to go all the way. Nothing white is the best pathway but it is very hard to find things with no sugar in them. Oatmeal seems to be about all you are left with since just about everything has some sugar in it. Some weeks that is ALL I eat. Oatmeal with a little salt and maybe some raisins. I really like food and my wife is an excellent cook who likes to cook so it is really painful to have to restrict the diet.

The principal defect in your thinking is lending any credence whatsoever to the orthodoxy of cholesterol.

The defect in the thinking of many people lies in focusing on weight or dietary restrictions such as corn, gluten, or carbs.

There is only one easy answer and if there’s any one “diet” to outline it, Paleo comes the closest but it, too, has severe shortcomings from where I sit.

Just for the sake of argument, keto is all BS.

People need to take the time to understand health. I hope to publish sometime in the next year to afford some help.

When my wife was diagnosed with breast cancer, she decided to lose some weight. The smart phone app MyFitnessPal was pressed into service. She went through 2 rounds of surgery and 5,000 RADs of radiation. A few days into her MyFitnessPal experiment, I added the app to my phone. I was 190 lbs, 5 ft 11 in. The app logs your daily weight and food intake 3 meals + snack. It also has a link to the Vivofit2/3 device to tally sleep and number of daily steps.

Today is Oct 7, 2018. My fitness diary shows a 1005 days "streak" of no-missed log days. I weigh 146 lbs now. 13.7% fat. 20.4 BMI. Satisfactory for a 62 year old. My last blood test results were perfect (except for the beta HCG level above 5). I'm 1985 cancer survivor and my "marker" is elevated again.

My "formula" derives from multiple approaches. The Zone diet...calories from protein 30%, from fat 30%, from carbs 40%. I follow the 5/2 diet. Monday/Thursday are 600 calorie days. Tu,Wed,Fr,Sat,Sun are 1250 calorie days. I weigh in each morning to the nearest 1/10th pound. Each meal is recorded.

Why so much effort? My maternal great grandfather was diabetic. My maternal grandfather was diabetic. My father was an insulin dependent type 2 diabetic (much alcohol consumption). My wife is diabetic (type 2). My Rat Terrier/Chihuahua is an insulin dependent diabetic. I'm not diabetic. I have no desire to join the club.

Is there a downside to being 5 ft 11 in and 146 lbs? A small one. I'm almost too light to kickstart my Yamaha SR400 motorcycle. Thankfully, the other bikes have electric start. The Harley Fat Bob at 870 lbs is no big deal to push around. My 3 big dogs in the 90+ lb range are a challenge.

My wife still maintains her MyFitnessPal and Vivofit4 records. She really has a sweet tooth and has difficulty even with the app helping track inputs. Her office mates are very unhelpful. Lots of goodies dropped into the dispatch office.

The one thing that doesn’t work is moderation.

My wife is a nurse and taught nutrition classes for many years. People used to stop us all the time when we were out and about and thank her for helping them to change their lives. This would be many years after she saw them last. She pushed nothing but common sense, moderation and developing healthy nutrition and exercise habits.

People do not stick with the radical crap. There is nothing worse for your health than getting into a cycle of losing and gaining weight repeatedly all while not getting any appreciable amount of real exercise. People that buy into nonsense are the primary reason average life spans in our country have actually started getting shorter.

While most of us eat more sugar than we should and more calories than we should... there is nothing wrong with whole grains such as corn and wheat as part of a healthy diet. Cutting out corn and wheat from your diet is unlikely to have much if any affect on your cholesterol or triglycerides.

Not true. I've met many skinny type 1 diabetics. Absent an insulin shot, the sugar lingers in their blood and damages organs. It doesn't get stashed into muscles and liver as glycogen. My Rat Terrier/Chihuahua cross became very fat, then suddenly lost weight and developed cataracts. He's a type 1 diabetic now. An insulin shot every 12 hours and careful monitoring of his blood glucose. We're keeping it around 100. His diet is very important. The "fat" dog food we were using for the other dogs was insufficient for him. He collapsed on the floor with his glucose down at 25. The vet brought him back with some glucose. He now gets his food very carefully monitored instead of eating from the "communal" bowls in the kitchen.

If you attempt to lower carbohydrate intake you should also raise fat intake proportionally. Otherwise you will likely fail to stay on the diet.

Largely, insulin resistance starts when ingested carb lectins mimic insulin. They plug into cellular insulin receptors without doing the job of insulin. They don’t transport blood glucose into clls. Cells starve, and the blood fills with glucose.

I did it!
Stopped 160 units of insulin Oct 1st. began with a dinner to dinner fast (24hours). Next, I tried a 36 hour fast and extended it to 45. In eight days I’ve dropped 9 pounds. My fasting glucose readings are around 125 mg. and non-fasting around 145mg. This with no insulin injections.
Google Dr. Jason Fung and understand how the treatment you’re receiving is sending you to an early grave

The really great thing is that as you do it, as time passes, fasting will get pretty easy.

Very little suffering.

Since I was the type who REALLY relishes eating, I was shocked to all h*ll at this.

After a month I was at One Meal A Day

I eat a single ketogenic meal between 5:00PM and 7:00PM. Brought my A1C back down to 5. Also losing about a pound/week.

I find that to be the case as well. After a few weeks, you simply quit craving food and you aren't hungry all the time.

Thank you for that lectin insight. I knew about them, but personally don’t seem to have any noticeable problem with them. However, someone I know might and this will be a step toward understanding those problems.

Nutrient groups:
1. Carbs
2. Fats
3. Protiens
Raise fats as you lower carbs. Shift totally to meat protein, and eat all you want. Just be sure to eat enough fat. Ghee is a good source.

I learned about how lectins cause type 2 diabetes directly from a couple of quality studies. Wheat germ agglutinin and bean concavalin A are notorious mimics of insulin.

My go to snack is now peanuts.

Actually, I stock up on those P3 Protein packs, that consist of nuts, cheese and chunks of ham/turkey/chicken. I carry a pack wherever I go whenever I get hungry. Beats a candy bar or a bag of chips.
