Welcome to the Marburg Surveillance Project.

This thread will be used for all of the latest Marburg Outbreak News and comments. This is the place to post all comments about the Marburg outbreak, all articles and links to articles about the Marburg outbreak.

We're going to use just one thread instead of having to go from article to article as we have in the past. We'll use this thread as long as we can.

May 4, 2005 U.S. Embassy, Office of Public Affairs

EMBASSY LATEST WARDEN MESSAGE ON THE HEMORRHAGIC FEVER IN UIGE PROVINCE, ANGOLA

The United States Embassy Luanda recommends against travel to Uige province, Angola due to an outbreak of Marburg hemorrhagic fever. As of May 4, 2005, the Ministry of Health reported 310 cases and 279 deaths over a five-month period. The Angolan Ministry of Health, the World Health Organization, and the Centers for Disease Control-Atlanta are cooperating to identify recommended public health measures and treatment.

As of May 4, 2005, there are no new case of Marburg hemorrhagic fever in Luanda, Cabinda, Kwanza Sul, Kwanza Norte and Malange provinces. There are no reports of people contracting the disease in those provinces, as well as there are no active cases now reported in those regions. In Uige province, 1 case and 1 death were only reported within Uige city. In Damba, Songo, Negage, Puri and Maquela do Zombo municipalities no new case was reported. Health officials recommend that normal sanitary precautions should be used, including frequent hand washing, to minimize any chance of exposure.

That press release is breathtakingly bad. It's numbers are out of date and it doesn't mention the city where they found 17 dead two days ago. State comes across as clueless.

Hi kawana,

Confirming that 90% death rate is observable. We need to confirm the mortality rate (derived from cases where patients received clinical care) this will give an early indication of virulence of this strain. Which due to the sheer numbers involved in this outbreak suggests it is different form previous strains.

Already done.

The mortality rate (defined as outcomes rather than a snapshot of statistics) is essentially 100%. That is, only 1 person of 255 has survived to date. All others have died within 10 days. Existing active cases simply have not died yet.

We have confirmed this by monitoring the reports over the last 6 weeks. There has only been the one exception.

Except we are assured there is a survivor.

Good God do you understand what that means? even Ebola Zaire is not that virulent. Please I have to see this with my own eyes do you have a source for those figures? I want to present them to my colleague in a few hours time, when I meet him for lunch.

The two disturbing features of this virus are the extended incubation period during which time the patient is infectious AND the apparent 100% mortality. Contact tracing is going to be very difficult.

For me the most disturbing thing, is why the patients recieving clinical care are not recovering 1 in 4 of them should be doing just that.

MSF Q&A on Mortality:

This outbreak seems to be much more lethal than past cases of Marburg.

It is very surprising. We will have to answer this question. It is possible that it has appeared very lethal because we have only had records of cases in a terminal stage and mainly among young children who are more vulnerable. Now that MSF team has reorganized the Uige isolation unit, it will allow the treatment of infected people in an early stage. The percentage of fatality could then hopefully decrease, although the case definition does influence the results

If you search the web you'll find the comment about the 1 released patient. Apologies, but don't have much time at work to look for it. There is only one official press release noting that 1 person survived. There could, of course, be other survivors that were not mentioned in the released--but that seems unlikely.

Ah yes thank you for this post, it seems that the measurements are not yet made. The corect metric is always a factor in determining virulence and mortality. This was precisely my point when i asked this question. i pray for the sake of those people that we see a drop in fatalities as a result.

Yeah, kind of gets your attention, doesn't it??

We do not have access to raw data on these threads, because we only can find the "reported" data from the WHO and the Angolan Health Ministry. They report only summary data.

Therefore, I cannot provide the raw data (listing by individual patient and outcome) that you require. I apologize.

The details of this mortality (i.e. 100% except for 1 case) was determined from the summary data. Several of the reports indicated that the survival rate was zero. The historical listing by EBH lists the threads, but I do not remember specifically where the reports are. However, I note that in this community (FReepers discussing Marburg), we had inferred this quite some time ago. Indeed, I was directed toward a listing of the standard epidemiological equations, and was able to simplify and re-derive them because of the 100% mortality boundary condition.

I have replotted the graphs. I am keeping the original growth plot, determined by regression to the earlier data (before "administrative reclassification"), and am also plotting the reported data. I will wait for one more data point before posting.

My general sense is that the reported data have been significantly undercounting the real infection rate. If that is the case, the reported results will slowly approach the fitted growth curve. Too early to tell. It could also be that the recent increases were an abberation. Another poster pointed out (forgive me, I forget who), that the SARS epidemic showed similar aberrant behavior.

I have a colleague who works with the CDC and WHO in this area of VHF. I will meet him today and see if he doesn't mind sending me his excel files containing the raw data. He is also an expert quantitative analyst. I suspect there will be pivot tables involved, if you like and there is no issue with providing third parties with this information, I will send the data in a spreadsheet. I suspect that may be a problem him even providing them to me. I have similar restrictions on data security for my influenza results when we send those to WHO and the CDC. until WHO and CDC make them public domain as a PDF we are usually unable to distribute the data.

For me the most disturbing thing, is why the patients recieving clinical care are not recovering 1 in 4 of them should be doing just that.

ALL of the infected medical personnel have died.

Past press reports put the number at 17, although yesterday I saw a reference upping the number to 19.

You would think that if anyone survived, it would be the medical people. That NONE have survived is quite significant.

Only one person is reported to have recovered, and that is questionable. No details were provided, not even how the person was treated. Nor did they address the question of whether the person was still contagious.

This is also much longer than an VHF outbreak should have lasted. But the stunning fatalities amongst the patient groups that have recieved clinical care is just well nothing short of staggering. words fail me

I guess I'm just over anxious to see your graphs now that the month of April has concluded. The first time I saw your graphs, it was a real eye opener for me. It illustrated so well how sharply the spread had increased in the more recent months, even with their revised numbers, as well as the potential for even greater spread of the disease. Your work is sincerely appreciated.
